Job Description
We are seeking an experienced and compassionate social worker to join a dedicated team focused on improving the quality of life for senior adults and individuals with disabilities. As a member of the Home and Community-Based Services (HCBS) team within the Department of Aging and Disability Services, you will play a key role in enhancing the independence and well-being of older adults and people with disabilities in the community.
Role Overview:
In this position, you will provide case management services and coordinate a variety of social services to eligible seniors and adults with disabilities. These services are designed to empower individuals to live independently in their homes and communities for as long as possible. Your work will focus on connecting clients with essential services such as transportation, meal preparation, housework assistance, personal care, and more.
Key Responsibilities:
Client Caseload Management: Oversee a caseload of clients, ensuring timely and effective support for older adults and their caregivers.
Home Visits & Assessments: Conduct home visits to assess eligibility for various programs and services, ensuring clients' needs are met.
Documentation & Compliance: Maintain accurate, complete, and timely documentation for each case while adhering to professional standards and regulatory requirements.
Stay Informed: Keep up-to-date with aging and disability issues, available resources, and local service providers to ensure clients receive the best possible care.
Cross-Training: Collaborate with other Aging and Disability programs to gain a broader understanding of available services and share knowledge across teams.
Additional Duties: Assist with other projects and tasks as directed by the program manager.
Qualifications:
The ideal candidate will have:
A Bachelor's degree in Social Work or a related field.
Experience in geriatrics and case management.
Strong verbal and written communication skills.
The ability to work collaboratively within a team setting.
Technology Requirements:
Proficiency in Microsoft Office and Adobe Acrobat is required.
Travel Requirements:
This position involves travel within a thirteen-county region. A valid driver's license and access to personal transportation are required.
Work Schedule & Flexibility:
This position is not remote. The selected candidate will be expected to work on-site during core business days. However, work-from-home flexibility may be offered on occasion, at the discretion of the manager.
Compensation & Benefits:
The starting pay will be based on your experience and qualifications.
Comprehensive benefits package including a fully paid pension plan and an optional 401(k) with a dollar-for-dollar match of up to 3% of gross salary.
