About the Role:

We are seeking an experienced and compassionate social worker to join our dynamic Home and Community Based Services (HCBS) team. In this role, you'll work alongside a dedicated group of professionals to improve the quality of life for senior adults and individuals with disabilities. Our goal is to help these individuals live independently within their homes and communities for as long as possible.

The HCBS team provides essential case management and coordinates a variety of social services for eligible seniors and adults with disabilities. You will play an important role in helping clients access necessary services, such as transportation, meal preparation, personal care, housework, and other supportive services.

Key Responsibilities:

Manage a caseload of clients to ensure timely and effective assistance for older adults and their caregivers.
Conduct home visits to assess client eligibility for various programs and services.
Ensure accurate and complete documentation in a timely manner.
Stay informed on aging, disability issues, available resources, and local service providers.
Maintain compliance with professional standards, program requirements, and industry regulations.
Cross-train with other programs within Aging and Disability Services as needed.
Assist with special projects and other duties as assigned by the program manager.

Qualifications:
A Bachelor's degree in Social Work or a related field.
Experience in geriatrics and case management.
Excellent verbal and written communication skills.
Ability to work collaboratively in a team setting.

Required Technology Skills:
Proficiency in Microsoft Office and Adobe Acrobat.

Travel Expectations:
This role involves travel across the region, so a valid driver's license and access to personal transportation is required.

Remote Work Flexibility:
This is not a remote position. The selected candidate will be expected to work onsite on core business days. However, some work-from-home flexibility may be available at the discretion of the manager.

Compensation:
Starting pay will be based on experience and qualifications.
We offer a fully paid pension through the Tennessee Consolidated Retirement System, along with an optional 401k with a dollar-for-dollar match of up to 3% of your gross salary. Additional benefits details are available upon request.
